We are excited to be a part of the new and enhanced Home Health Quality Improvement (HHQI) National Campaign. As the contractor for the first HHQI Campaign, WVMI & Quality Insights is proud to work again with this initiative of Centers for Medicare & Medicaid Services (CMS) to improve home health quality.
Our campaign goals include improving the rate associated with acute care hospitalizations (reducing avoidable hospitalizations) and management of oral medications for home health patients. Although the campaign is led by the home health community, we are forming partnerships with other provider groups such as hospitals and physicians’ offices. At the focal point of our educational efforts is new multimedia Best Practice Intervention Packages (BPIPs) designed to educate home health agencies. These free resources are available to all participants who register and will be released at regular intervals.
Participants and supporters also can access free online sharing tools designed to facilitate the free flow of information among stakeholders. These channels are designed to unite providers across settings under the shared vision of reducing avoidable hospitalizations and improving medication management.
